How can I make an appointment?
You can schedule an appointment by phone during our office hours. Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Or you can can request an appointment online and one of our team members will respond to your request within 24h to schedule your appointment.
How much does a consultation cost?
Treatment costs depend on the visit's length and difficulty. Prior to the commencement of treatment, fees and financial arrangements will be discussed with you. Feel free to call us anytime if you would like an estimate.
Do you offer Insurance?
We have experience working with the majority of dental health plans. We are always available if you have questions about your dental plan.
Where is the practice located?
Our office is conveniently located in Los Alamitos on Cerritos Avenue, and easily accessible from Los Alamitos, Cypress, Long Beach and Buena Park.

Do I need to arrive early for my first appointment?
Yes. Please arrive 10-15 minutes early to fill out any remaining patient forms.
What payment options do you offer?
In order to provide the highest quality care on a sound basis, we have developed a full range of convenient payment options for you to choose from. We can coordinate financial arrengements with your dental insurance benefits. Major credit cards such as Visa and MasterCard are accepted in our office. We can also help you arrange outside financing if necessary.
Please bring the following items with you to your appointment:
- Patient Information Form
- Dental Insurance Card (if applicable)
- Identification such as Driver's License, Military ID or State ID
- Patient Health History Form
- HIPAA Consent Form
- Patient Authorization Form
Dental anxiety or phobia is real and we see a lot of patients with this condition. We spend a lot of time finding what causes this anxiety and address it accordingly. We offer nitrous oxide commonly called laughing gas, headphones, blankets, etc to help patients ease their anxiety. Please, don't be afraid to voice your anxiety. We are ready to listen and help.
